Frequently Asked Questions About Your Care
How much does GLP-1 therapy cost at Everwell, and how does it compare to online programs like Ro or Hims?
Most online telehealth platforms advertise low introductory rates, but tack on hidden monthly membership fees ($99–$149/month) and significantly increase the cost of medication as your dose goes up. At higher maintenance doses, many online programs charge $400 to $600+ per month all-in.
At Everwell Internal Medicine, we believe in transparent, physician-led care without inflated markups or hidden subscription tiers. Because we pass wholesale pricing directly to our patients and eliminate recurring telehealth "tech fees," our all-in costs remain significantly lower across the entire treatment course
All in cost
Semaglutide
Starting dose $257 a month
Max dose $313 a month
Tirzepatide
Starting dose $270 a month
Max dose $373 a month
At Everwell Internal Medicine you get significantly lower out-of-pocket medication costs compared to online glp1 weight loss programs PLUS full membership Internal Medicine included:
- Unbeatable Total Cost: Because we pass direct pricing to you without corporate markups, our all-in cost for both Semaglutide and Tirzepatide remains hundreds of dollars less per month than big telehealth brands—at both starter and max maintenance doses.
- Full Membership Physician Care: Unlike online services that manage weight loss in isolation, your treatment is fully integrated into a dedicated concierge doctor-patient relationship. You receive direct physician access, personalized lab tracking, tailored dosing, and comprehensive whole-body primary care all under one roof.
What’s different about MDVIP practices and traditional primary care practices?
Doctors who operate in the MDVIP models typically see far fewer patients than traditional primary care doctors. That gives them more time to develop deeper doctor-patient relationships that can lead to better outcomes. They can also offer conveniences that most primary care doctors can’t, including same- and next-day appointments. They’re available after hours for urgent matters. And they have time to focus on prevention. That focus begins with the MDVIP Wellness Program, which patients pay for with their annual membership fee.
How does insurance work in an MDVIP-affiliated practice?
Most MDVIP-affiliated primary care practices accept insurance (your physician can tell you whether they accept your specific insurance plan). Your annual fee pays for preventive care medical services that insurance usually doesn’t cover (e.g., advanced diagnostic testing and screenings). Your MDVIP-affiliated physician will continue to bill your insurance and charge copays, co-insurances and deductibles as he or she currently does for other medical services like sick visits.
What is MDVIP and what does it do?
MDVIP makes wellness and prevention easier by providing your physician with innovative tools and technology. MDVIP supports your physician’s practice and seeks to eliminate barriers to a positive healthcare experience. Your doctor will determine whether and how to incorporate these tools at his/her discretion.
What specific blood and urine tests are included in the MDVIP Annual Wellness Exam?
- TMAO (Trimethylamine N-oxide): Measures a compound produced by gut bacteria when digesting specific dietary fats (like red meat and egg yolks). High TMAO levels directly drive arterial plaque accumulation and blood clotting risk, independent of your cholesterol.
- Apolipoprotein B (ApoB): Counts the exact number of plaque-causing (atherogenic) particles in your bloodstream. Every bad lipoprotein molecule carries exactly one ApoB protein, offering a far more accurate cardiovascular risk assessment than standard LDL weight measurements.
- Lipoprotein (a) or Lp(a): Tracks a highly sticky, genetically determined variant of LDL cholesterol that aggressively accumulates on arterial walls to drive premature coronary plaque development.
- LDL Particle Number (LDL-P) & Small, Dense LDL: Analyzes individual LDL and HDL particle counts and physical sizing rather than just measuring total weight, identifying hidden risk patterns like tiny, highly atherogenic sdLDL particles.
- Standard Lipid Baseline: Captures absolute values for Total Cholesterol, HDL ("good") Cholesterol, LDL ("bad") Cholesterol, Triglycerides, and the Total/HDL Ratio.
- High-Sensitivity C-Reactive Protein (hs-CRP): Identifies low-grade, systemic baseline inflammation actively irritating arterial walls.
- Myeloperoxidase (MPO): Tracks white blood cell activation inside arteries, indicating whether arterial plaque is unstable and vulnerable to tearing.
- Lp-PLA2 Activity (PLAC® Test): Measures the inflammatory enzyme actively produced inside arterial wall plaque.
- Homocysteine: Tracks direct endothelial wall irritation, cellular methylation, and clotting risks.
- Fibrinogen Activity: Evaluates blood viscosity and systemic inflammatory clotting markers.
- Urine Microalbumin/Creatinine Ratio (uACR): Rather than a basic urinalysis, we run a precision microalbumin screen. Microscopic protein leaks in the urine are a direct reflection of microvascular damage, serving as an early-warning radar for kidney health stress and systemic endothelial dysfunction across your entire vascular network.
- Cardio IQ® Insulin Resistance Score: A calculated score utilizing Fasting Insulin, Fasting Glucose, and C-Peptide to isolate and identify underlying cellular insulin resistance years before your standard blood sugar levels begin to rise.
- Hemoglobin A1c (HbA1c): Measures average blood sugar over a 60-to-90 day window to identify early glycemic trends.
- Uric Acid: Evaluates metabolic byproduct concentration to screen for systemic stress, gout indicators, and endothelial dysfunction.
- Kidney Function: Blood Urea Nitrogen (BUN), Creatinine, and eGFR (Estimated Glomerular Filtration Rate).
- Liver Enzymes & Proteins: Alanine Aminotransferase (ALT), Aspartate Aminotransferase (AST), Alkaline Phosphatase (ALP), Total Bilirubin, Albumin, and Total Protein.
- Electrolytes & Fluid Balance: Sodium, Potassium, Chloride, Carbon Dioxide, and Calcium.
- Red Blood Cells & Oxygen Markers: Red Blood Cell (RBC) count, Hemoglobin, Hematocrit, and RBC Indices (MCV, MCH, MCHC, RDW) to screen for anemia types.
- Immune Status: White Blood Cell (WBC) count and relative percentages (Neutrophils, Lymphocytes, Monocytes, Eosinophils, Basophils).
- Clotting Function: Total Platelet Count.
- Vitamin D (25-Hydroxy) & Vitamin B12: Assesses global hormone, immune system, nervous system, and cellular bone health baselines.
- Thyroid Stimulating Hormone (TSH) & Free T4: Evaluates central and circulating regulation of your baseline metabolic speed.
Who sees me during my appointments? Will I ever be seen by a Nurse Practitioner or Physician Assistant?
Never. Dr. Rosenstein personally conducts every single consultation, diagnostic review, and annual wellness exam. Because we maintain a strictly capped patient roster, we do not utilize mid-level providers (NPs or PAs) to offset high patient volume. You get 100% of Dr. Rosenstein's 20+ years of clinical and academic expertise at every visit.
Does Dr. Rosenstein have Admitting Privileges if I am taken to a Las Vegas area hospital
In the modern healthcare system, acute inpatient care is managed directly by the on-site hospitalist team.
While Dr. Rosenstein does not admit patients or directly manage your hospital care once admitted, his background as a hospitalist for 10 years means he collaborates directly with your inpatient doctor/care team and is involved in your care every step of the way.
When patients are specifically admitted to Mountain View Hospital, Dr. Rosenstein directly partners with specific hospitalists who he’s both hired and worked with for over 10 years, to make sure the highest quality care is delivered.
His patients are instructed to let him know immediately upon arrival at the hospital so that if his specific preferred physicians can be assigned to the case, they will be. Mountain View is his preferred hospital of choice for patients to go to because of his ability to reach into the institution and assist with the care.
Does the practice offer virtual visits or text messaging with Dr. Rosenstein?
Yes. We understand that your time is valuable. While initial comprehensive evaluations and advanced biometric reviews are best conducted in our Summerlin office, routine follow-ups, laboratory reviews, and minor acute concerns can be handled seamlessly via secure telehealth video visits.
Additionally, as an active member of our practice, you have Dr. Rosenstein’s personal cell phone number allowing you to bypass frustrating corporate call centers and even his own staff.
Can I access my advanced lab results and medical records online?
Absolutely. Our practice provides a secure, fully encrypted online Patient Portal accessible 24/7 from your computer or smartphone. Every advanced test we run—including your Cleveland Heart Lab vascular inflammation metrics, ApoB particle counts, and Cardio IQ® Insulin Resistance scores—is uploaded directly to your portal along with Dr. Rosenstein's clinical notes, allowing you to track your longevity trends over time.
At what age can someone join Dr. Rosenstein's Internal Medicine practice?
As a specialist in adult Internal Medicine, Dr. Rosenstein accepts and manages patients starting at 18 years of age. We frequently welcome young adults transitioning out of pediatric care, allowing them to establish a foundational, lifetime baseline for metabolic fitness, cardiovascular healthspan tracking, and disease prevention early in their adulthood.
If I already see multiple specialists, how does Dr. Rosenstein interact with them?
I'm a First Responder - How does Dr. Rosenstein’s 11 years of experience caring for First Responders translate to specialized care for my occupation?
- Confidential, Off-The-Record PTSD Support: The repetitive, acute trauma faced by first responders leads to disproportionately high rates of Post-Traumatic Stress Disorder (PTSD) and chronic neurological burnout. We understand the immense professional pressure regarding fitness-for-duty metrics and the fear of career repercussions. Because we operate an independent, private boutique practice with a strictly capped panel, Dr. Rosenstein provides a safe harbor. He is completely willing to manage, counsel, and treat PTSD confidentially and off-the-record, ensuring you receive elite mental healthspan support without jeopardizing your badge, rank, or livelihood.
- Proactive Testosterone & Hormone Optimization: Chronic shift work, sleep deprivation, and sustained high-cortisol adrenaline spikes completely devastate the endocrine system, causing first responders to suffer from severely low testosterone levels. This leads to persistent fatigue, loss of lean muscle mass, systemic inflammation, and depression. Dr. Rosenstein is highly proactive about tracking and correcting low testosterone, designing evidence-based hormone optimization protocols to safely restore your physical strength, metabolic drive, and vitality.
- Advanced Cardiovascular Defense: The sudden hyper-sympathetic adrenaline surges faced in the line of duty dramatically accelerate arterial plaque accumulation and heart disease risk. Dr. Rosenstein bypasses standard cholesterol checks, utilizing ApoB, TMAO, and Cleveland HeartLab vascular inflammation panels (hs-CRP and MPO) to catch and treat hidden arterial inflammation years before it can trigger an acute cardiac event.
How does Dr. Rosenstein's 20+ years practicing medicine in Las Vegas benefit my care if I need to see a specialist quickly?
- We Bypass the Waiting Lists with Close Personal Relationships: Over a quarter-century of training and working extensively inside local hospitals, Dr. Rosenstein hasn't just memorized a directory of names—he has worked shoulder-to-shoulder with the finest physicians in the city. Because he has close personal relationships with top local specialists, our office can directly coordinate your care. When you need a cardiologist, oncologist, neurologist, or surgeon, you aren't treated as a random insurance referral; you get a direct, trusted introduction from a peer they have known for decades.
- We Know Who is Truly Excellent: There is a massive difference between a doctor who ranks well on a basic insurance portal and a doctor who possesses elite clinical skill. Because Dr. Rosenstein spent ten years managing complex, acute cases as a hospitalist in the local hospital networks, he knows exactly who the premier, evidence-based specialists are based on real-world clinical performance. He directs you exclusively to physicians who match his own rigorous standards for longevity and preventative medicine.
How does Dr. Rosenstein's complete clinical independence directly protect my health interests?
Can Dr. Rosenstein prescribe unapproved wellness peptides? How can the practice help me navigate peptide therapy?
- Translating the Science: If you are exploring or currently utilizing emerging peptide therapies, Dr. Rosenstein can provide an exhaustive, evidence-based breakdown of how these specific amino acid chains interact with your cellular pathways, metabolic systems, and tissue-repair mechanisms.
- Comprehensive Biomarker & Safety Monitoring: Operating in the "gray market" or ordering unregulated peptides online exposes you to significant risks, including inconsistent dosing and zero quality control. Dr. Rosenstein can order and interpret essential baseline labs—such as IGF-1, Fasting Insulin, and HbA1c—to monitor your physiological markers, screen for contraindications, and actively look out for adverse side effects.
- Fully Approved Alternatives: For fields like metabolic and body function optimization and tissue healing Dr. Rosenstein can design compliant, highly effective treatment blueprints utilizing fully FDA-approved therapeutics (such as standard GLP-1 receptor agonists, growth hormone analogs, or advanced physical medicine modalities like PRP).
Do you provide proactive healthspan optimization alongside standard lifespan care?
Yes. At Everwell Internal Medicine, our medical architecture is engineered entirely around healthspan optimization. By tracking early vascular, cognitive, and metabolic trends through the lens of active clinical trial data, Dr. Rosenstein designs custom exercise, nutritional, and therapeutic protocols to preserve your peak physical agility, cognitive sharpness, and cellular vitality as you age.
What exactly is the difference between lifespan and healthspan and why should it change how you practice medicine?
How does Dr. Rosenstein's active involvement in FDA-approved Clinical Trials translate to better care for me?
- We See Tomorrow's Medicine Today: Working directly with experimental protocols gives Dr. Rosenstein firsthand insight into the safety, efficacy, and real-world mechanisms of emerging therapeutics years before they hit the commercial market. This deep, early familiarity allows him to optimize your preventive protocols with unmatched foresight.
- A Culture of Extreme Clinical Precision: Clinical trials demand a hyper-rigorous, meticulous standard of data collection, patient monitoring, and biomarker tracking. Dr. Rosenstein brings this exact data-driven discipline out of the lab and straight into your private 60-minute consultations. Every variable of your health—from your gut microbiome to your vascular inflammation—is analyzed with scientific precision.
- True Evidence-Based Personalization: In a trial setting, you quickly learn that patients are not statistics; different biologies respond uniquely to the exact same treatment. Dr. Rosenstein rejects generic, assembly-line medicine. He utilizes his trial background to evaluate your specific genetic, metabolic, and cellular markers, tailoring a highly individualized longevity plan built on the most current active literature.
